Sadly for most of these recommendations there are little data. Some of these recommendations may be somewhat controversial - for example saying hospitals should routinely consult palliative care or ethics and that they should have futility policies.
In the country where I practice, there is a similar national policy from our registration body that covers many of these points in the context of our legal rules. So, this overlaps but cannot replace the statutory policy of legal binding. For example, terminal extubation is not allowed in certain cases such as trauma or likely procedure-related mortality.
